  • For Partnerships:
    • Two (2) valid IDs of the managing partners
    • SEC Certificate of Registration with signed application

  • For Single Proprietorship:
    • Two (2) valid IDs of the business owner
    • Original copy of unexpired DTI Certificate of Registration
    • If the spouse is designated as co-signatory:
      • Two (2) valid IDs of the spouse
      • Written authorization of the registered business owner allowing his/her spouse to act as co-signatory

Notes:

  • Memorandum of Agreement is a requirement for all business types (Corporation, Partnership and Sole Proprietorship).
